New Client Forms

If you are coming to see me for the first time, you may download the forms below.   Fill them out at your leisure and bring them to your first session.   This will save us time for reflection together rather than filling out paperwork.  

Please download the 3 documents immediately below.   Read the information carefully, especially the "Informed Consent" document.  You may wait until we meet to sign the "Informed Consent" so that you can ask me questions about its content.

For those of you entering supervision of psychotherapy and counseling with me you will find copies of an information sheet as well as informed consent below.  Please make certain you are downloading the correct informed consent for supervision.  The first Informed Consent is for private supervisees; the second is for graduate counseling students at Mercer University.
